SCHOLARSHIP PROGRAM

Welcome to the Attleboro White Hawks! It is our hope that we can help as many people as possible through our scholarship program. A few pieces of information that are important:

• Please note that all scholarships are awarded on the basis of need – this application as well as proof of income (copy of all W2s from 2016, child support, etc.) will be required of all applicants. While we wish we could help everyone, not all applicants are guaranteed a scholarship.

• To help as many as possible, we reserve the right to award full and partial scholarships.

• Scholarships will cover registration fees only – all miscellaneous costs of participation, including equipment not provided, will be the responsibility of the parent/guardian.

• All scholarship recipients will need to sell the required raffle tickets.

• Parents/Guardians will be required to volunteer a designated number of hours on game day – volunteer requirements will be based upon the scholarship awarded and failure to fulfill your requirements will jeopardize future scholarships.

Box 3344, Attleboro, MA 02703. Please feel free to black out social security numbers however names/addresses must remain.

SCHOLARSHIP APPLICATION

Everyone, please take a moment to familiarize yourself with Pop Warner's and the Attleboro City Recreation Department's Code of Conduct policy regarding smoking during practices, games and all league events.

Pop Warner’s Adult Code of Conduct S2:
“Any adult who is using alcohol, tobacco or non-prescription drugs and/or appears intoxicated at a Pop Warner event, and/or who is flagrantly rude, attempts to intimidate, verbally abuse, heckles, taunts, ridicules, boos, throws objects and/or uses vulgarity or profane language/gestures with an official, coach, volunteer, staff member, participant or other event attendee, must receive a verbal warning and/or be asked to leave a Pop Warner event. The member organization may also provide a written warning to the individual regarding the misbehavior.”

City of Attleboro Recreation Department’s Code of Conduct Policy #4: “...inappropriate behavior defined as including but not limited to: the possession, use or consumption of alcoholic beverages or illegal substances, including marijuana. The use of smoking products including cigarettes and chew”

This policy can also be found within the Player Contract that each participant and parent/guardian has signed.

Simply put, the use of smoking products, including cigarettes, marijuana, chew and e-cigarettes is strictly prohibited in parks, fields and all recreational facilities.

Please refer to the maps for Lee’s Pond and Hayward/Walsh fields where smoking is prohibited anywhere inside of the green border, including field parking lots.
